  Includes registration, medical history, insurance, and consent forms for new patients. Fill out online or download a blank printable version.

• Fill Out Online — Adult Patient Packet Download Blank Adult Patient Packet (PDF)

  For existing patients turning 18 — includes adult consent for treatment, assignment of benefits, health profile, and authorization to communicate forms.

• Fill Out Online — Patient Update Packet Download Blank Patient Update Packet (PDF)

  For existing patients — complete every 3 months to keep your information current. Includes contact updates, parent substitute authorization, HIPAA acknowledgement, informed consent, assignment of benefits, and vaccine policy.

No-Show Policy

We understand that schedules can change unexpectedly. However, to ensure availability for all patients, please be aware of the following:

• If you arrive more than 15 minutes late for your scheduled appointment, you may need to reschedule.
• If you fail to show for your appointment or cancel without at least 24 hours notice, you may be required to pay a small nominal fee that is not covered by insurance.
